We encourage participation from primary care medical providers taking part in APM 1\, as well as all Colorado stakeholders with an interest in investments in primary care\, performance and accountability\, and HCPF plans for value-based payment reform. \nMeetings are available by remote participation only. Please register for each of the three sessions to receive details and materials prior to the meetings. All registrants will also receive a link to a recording of an introductory webinar providing background on APM 1 to review prior to attending the first session. \nThe Colorado Health Institute (CHI) will convene and facilitate each meeting. CHI is accepting written feedback or questions in advance at focusgroups@coloradohealthinstitute.org   \nAuxiliary aids and services for individuals with disabilities and language services for individuals whose first language is not English may be provided upon request. Please notify Kelly O’Brien at 303-868-4983 or kelly.obrien@state.co.us or the 504/ADA Coordinator at hcpf504ada@state.co.us at least one week prior to the meeting to make arrangements. \n  \n
